  3. Written Summary & Recommendations
    You will receive a clear plan outlining equipment, safety considerations, and caregiver support strategies.

  4. Coordination & Follow-Up

    We ensure recommendations are understood and that clients and caregivers feel fully prepared for the transition home.

  • No, you do not need to prepare. We review your home as it is and guide you step by step through the next recommendations.

  • Yes, caregivers are encouraged to participate. Their involvement ensures they feel confident and capable in providing safe support at home.

  • Ideally, booking a few days before discharge allows time to complete the assessment and put recommendations in place. However, we also provide support after you return home.
