Assistive Devices Program (ADP) Assessments
Access government funding for mobility aids.
Accessing government-funded mobility equipment doesn’t have to be overwhelming. Our occupational therapists provide comprehensive ADP assessments to help you secure funding for walkers, wheelchairs, seating systems, and other assistive devices.
During the assessment, we review your eligibility, mobility needs, and daily routines to recommend equipment that supports safety, independence, and comfort. We also handle the paperwork, coordinate with approved vendors, and follow up to ensure your equipment meets your needs long-term. This service is especially helpful for clients on ODSP or OW, where funding may cover up to 100% of the approved equipment cost.
With an ADP assessment, you gain professional guidance, reduced financial stress, and peace of mind knowing your application and equipment selection are handled thoroughly.
Home Safety Assessments
Reduce fall risk and support independence at home.
Falls are one of the leading causes of injury among adults at home, but many risks can be prevented with careful planning. Our occupational therapists conduct thorough assessments of your living space, reviewing areas such as bathrooms, stairs, bedrooms, and common areas. We identify hazards, recommend modifications, and provide strategies to make daily routines safer.
During your assessment, we consider your mobility, cognitive abilities, and lifestyle to suggest practical changes—such as grab bars, improved lighting, non-slip surfaces, or safer furniture arrangements. Our recommendations aim to reduce fall risk, promote independence, and support caregivers in their daily routines.
Home Modification Support
Plan safe and practical home changes.
Some clients require larger changes to their living space to remain safe and independent. Elevate Mobility’s occupational therapists provide hands-on guidance and documentation for home modifications such as ramps, stair lifts, and accessible bathroom renovations.
We work closely with your healthcare providers and funding programs like March of Dimes to advocate for solutions that are both safe and cost-effective. From planning to implementation, we focus on changes that enhance accessibility, reduce fall risk, and support daily activities, so you can continue living confidently in your home.
Equipment Recommendations

Choosing the right mobility or assistive equipment can dramatically improve safety, comfort, and quality of life. We guide you through all available funding options, including the Assistive Devices Program (ADP), and coordinate directly with trusted vendors. Cognitive Assessments

Hospital-to-Home Transitions

Returning home after a hospital stay can feel overwhelming. Our OTs work closely with you, your family, and your care team to ensure your home is ready for a safe return. We coordinate necessary equipment, recommend environmental adjustments, and provide education for both clients and caregivers.
From assessing your ability to perform daily tasks to arranging mobility aids and home supports, we help prevent setbacks and reduce the risk of readmission. Our goal is a smooth, confident transition from hospital to home that prioritizes safety and independence.
Education & Support for Caregivers
Practical guidance to make daily care safer and easier.
Caring for a loved one can be rewarding, but it also carries physical and emotional demands. Our occupational therapists provide personalized, in-home training for caregivers, focusing on safe transfer techniques, injury prevention, and practical strategies to make daily care easier.
Education is tailored to your home environment and the unique needs of the person you support. By learning how to balance safety with independence, caregivers gain confidence while helping their loved one maintain participation in meaningful activities.

Direct Billing for ODSP Clients
We offer direct billing for eligible ODSP clients with pre-approval from your case manager. Our team guides both you and your case manager through the process to make OT services as accessible as possible.
