All assessments are completed by a registered occupational therapist — in the comfort and privacy of home.

Home Safety Assessments
Reduce fall risk and support independence at home.
Elevate Mobility provides comprehensive in-home safety assessments to reduce the risk of falls and support independence at home. We evaluate your living environment, identify safety concerns (e.g., bathroom access, stairs, entryways), and provide practical recommendations tailored to your needs. A detailed written report is included, and assessments can be used to support applications to programs such as March of Dimes.
Hospital-to-Home Transitions
Practical planning to prevent setbacks after leaving hospital.
We help ease the transition from hospital to home by ensuring necessary mobility equipment and home modifications are in place before discharge. We can liaise with vendors and collaborate with your care team to expedite installation of ramps, grab bars, bed rails, and more. Our goal is to reduce hospital readmissions and ensure a smooth, safe return home.
Equipment Recommendations & Vendor Coordination
Get the right equipment without the guesswork.
We provide recommendations for mobility and home safety equipment based on clinical needs, safety concerns, and ease of use. Following the assessment, we prepare a detailed report that can support collaboration with vendors and care teams to guide appropriate selection and placement of items like grab bars, transfer aids, or raised toilet seats.
Our goal is to ensure each client receives the equipment that best supports their safety and independence—without the guesswork.
Cognitive Assessments
Capture your current thinking skills to guide care and track changes over time.
Cognitive assessments provide a clear picture of your current level of thinking and memory skills, including attention, problem-solving, and daily functioning. This helps identify strategies to support independence and safety at home. A major benefit is having a baseline record of your current cognitive performance, which can be used to track changes over time, monitor disease progression, or guide future care planning. A written summary with practical recommendations is included, and results may support applications for funding or community programs.
Home Modification Support
Plan safe and practical changes to your home.
When larger home changes are required—such as ramps, stair lifts, or bathroom renovations—we can assist in beginning the application process with organizations such as March of Dimes. We provide clear, professional reports and advocacy to support funding requests and collaborate with your family doctor or nurse practitioner to help move the process forward.
Assistive Devices Program (ADP) Assessments
Access government funding for mobility aids.
We are authorized to complete ADP applications for walkers, manual and power wheelchairs, and seating components such as cushions and backrests. If you are eligible, we assess your needs, recommend appropriate equipment, and guide you through the application process for government-funded mobility devices. We also support requests for replacements or upgrades.
Education & Support for Caregivers
Practical training to make daily care safer and easier.
We offer personalized training and education to family members, private caregivers, and support staff. These sessions are designed to improve safety, confidence, and quality of care at home. Topics may include:
• Safe transfers to reduce the risk of injury for both the caregiver and client
• Pressure management to protect skin integrity and reduce the risk of pressure injuries
• Encouraging engagement in daily routines and meaningful activities
Training is tailored to each client’s needs and can be delivered in-person during home visits. We aim to empower families and caregivers with practical strategies that support comfort, safety, and independence.
