Our occupational therapists support adults in Orléans with home safety assessments, equipment recommendations, ADP assessments, and caregiver training. Services are provided directly in your home, helping reduce fall risks, promote safe mobility, and make daily routines easier.

Occupational Therapy Nepean

In Nepean, Elevate Mobility provides hospital-to-home transitions, home modification planning, and equipment guidance. Recommendations are tailored to each home environment so clients and caregivers can feel supported in daily activities.

We bring occupational therapy services to Kanata with a focus on fall prevention, safe transfers, and ADP funding support. From recommending mobility aids to planning home modifications, our goal is to provide practical, sustainable solutions.

Occupational Therapy Barrhaven

In Barrhaven, our occupational therapists complete in-home assessments that may include strategies for bathroom safety, stair navigation, or energy conservation. Equipment such as shower chairs, raised toilet seats, and grab bars can also be recommended.

Occupational Therapy Gloucester

Gloucester residents benefit from home safety and cognitive assessments that capture current needs and help plan for the future. Caregiver education is also available to ensure safe support in the home.

Occupational Therapy Stittsville

Our occupational therapy services in Stittsville help adults stay safe and supported at home through equipment recommendations, caregiver training, and funding guidance.

Occupational Therapy Cornwall

In Cornwall, our occupational therapists provide assessments that may support applications to the Assistive Devices Program (ADP). Clients may be eligible for 75% coverage, or 100% coverage if enrolled in ODSP or Ontario Works.

Occupational Therapy Chesterville

Chesterville clients receive home modification support, safety assessments, and equipment recommendations to promote safe participation in meaningful activities and reduce caregiver strain.

Occupational Therapy Morrisburg & Iroquois

We also travel to Morrisburg and Iroquois to provide occupational therapy services including hospital discharge planning, home safety strategies, and caregiver education.

What to Expect

Our process is simple, respectful, and client-centered.

  1. Initial Phone Call
    We’ll discuss your needs and make sure we’re the right fit.

  2. In-Home Assessment
    A licensed occupational therapist visits your home to complete the evaluation.

  3. Equipment Recommendation
    We guide you through options and determine whether funding (ADP, ODSP, OW, or private insurance) is available.

  4. Funding Application & Documentation
    We complete all necessary forms, coordinate with vendors and physicians or nurse practitioners, and liaise with caseworkers when needed.

  5. Follow-Up
    We follow up by phone or email to answer questions and ensure everything has been received.
